REQUESTING YOUR MEDICAL RECORDS

According the law "Patients rights", as a patient, you can obtain, with some exceptions*, a copy of your medical file : 

This right can also be exercised by a third party acting on behalf of the patient : either the "trusted person" designated by the patient, or the patient's legal representative.  If the patient is a minor and/or unable to exercise his/her rights himself/herself.  For minors, a copy of the identity card of the children and of the requesting parent or legal quardian(s) is mandatory.

Files will be delivered, by appointment, on Mondays, Tuesdays, Thursdays and Fridays between 9 and 11 a.m., maximum 15 working days after the written confirmation of the request.

Postal delivery in Belgium is exceptional and will be sent by registered mail with acknowledgement of receipt.  In this case, the Europe Hospitals decline all responsibility for the possible loss of documents.  In exceptional cases, it is possible to send documents by secure electronic means via a transfer platform. 

If the patient is deceased, no copy of the medical records may be issued or sent.  After the death of the patient, the records may only be consulted through a doctor designated by the family of the deceased subject to a reasoned request by them.  Send requests to the legal department (n.bourlez@europehospitals.be).

PLEASE NOTE :  for security reasons, we do not send files by email

The Medical Records Department does not deal with requests from the medical departments of other hospitals or from insurance companies requesting justification of hospitalisation for payment of bills ; these should be sent tot the consultation secretariat of the department or medical speciality which treated the requesting patient.

Requests for copies of medical files in the context of a medical expertise should be addressed to the legal service of the institution .  Any information can be asked by calling the secretariat at 02-614 27 91.

*Article  9 § 3 of the law of 22 august 2002 : A professional practitioner may refuse to issue a copy of the medical records if he/she has clear indications that the patient is under pressure to hand over a copy of their file to third parties.
