Request for appointment cancellation

If you cannot honour your scheduled appointment, we request that you release the time slot by notifying us at your earliest convenience, at least 24 hours in advance (48 hours for nuclear medicine). Otherwhise, a 25 € fees will be charged to you.
To cancel an appointment, you can fill in and send the online form below or you alternatively may use our phone line: 02-614 49 99 from Monday to Friday between 8am and 6pm.

To cancel an appointment at the Bella Vita Medical Center, please send an email to secretariat@bvmc.be or you can call on 02-614 42 00. 

This online form is exclusively used to cancel your appointment! To book a new appointment, please contact the department concerned. Please note that booking appointments online will not be taken into consideration! 
If you wish to cancel several appointments, please fill in the form repeatedly.
