The clinic

We emphasise that heart failure is a serious chronic pathology, with a high mortality. This means that it requires long-term treatment. Therefore, adequate care is essential, in order to reduce the duration and frequency of hospitalisations while improving the patients' quality of life.

The heart failure clinic provides a programme of care and follow-up for heart failure patients in the hospital and on an ambulatory basis within the two sites of the Europe Hospitals (St-Michel site and St-Elisabeth site) by a dedicated medical team. The main objective of this team is to create an interdisciplinary partnership so that the patient becomes, in the long term, the major actor of his disease (self-detection of warning signs, appropriate reaction...).

As soon as the diagnosis is established, as well as throughout the duration of their illness, patients with heart failure will benefit from a multidisciplinary follow-up programme provided by our 4 cardiologists and our 2 specialist nurses as well as the rest of the paramedical team.

In practice :

Who can be followed at the heart failure clinic ?

Any person diagnosed with heart failure or any patient with symptoms suggesting heart failure (breathlessness, swelling of the legs, weight gain, fatigue, etc.).

The patient can either make an appointment or be referred by their general practitioner or cardiologist.

How are heart failure patients monitored ?

In practice, we carry out specialised consultations, at individualised frequencies and coordinated by the referring cardiologist and the heart failure nurse.

Our heart failure nurses take some basic parameters and provide health education, including evaluation of the patient's level of information and involvement, identification of the factors favouring cardiac decompensation, explanation of the benefits of treatment, the importance of weight control and a low-salt diet, etc.

The cardiologist ensures the complete clinical evaluation (electrocardiogram, blood biology control, cardiac echography...), verifies the effectiveness and the good tolerance of the treatment as well as its adaptation. Other investigations or treatments are carried out in relation to the pathology. He also ensures that all information is communicated to the patient's general practitioner.

We also carry out ergospirometry (cardiopulmonary effort test) which allows us to better evaluate the heart's reserve during an effort.

For eligible patients, our accredited cardiac rehabilitation centres (on the St-Michel site and on the St-Elisabeth site) offer a cardiac revalidation programme that is complementary to conventional treatment. An adapted and personalised programme is carefully elaborated by our two cardiologist rehabilitators and provided by our specialised physiotherapists with the aim of improving the overall health of our heart failure patients.

The Heart Failure Clinic offers a personalised outpatient diet programme which we strongly recommend to patients in order to regain a certain autonomy in their dietary choices.

Our teams of social workers and psychologists are also involved in the care of the patient.
